Referrals

Living with a sensory loss can impact on many aspects of day to day life. Here at VisionPK our aim is to offer the right help and support for people to live as independently as possible. 

Our expert team is here to offer advice and practical help as well as working with other organisation’s and services to ensure that individuals receive the support they need both now and in the future. 

To make a referral, you can self refer by filling in our referral form below or a family member or friend can do this for you. Alternatively you can ask any other health and social care professional to refer you e.g. your GP, Optician, Occupational Therapist, Social Worker etc. 

If you are making a referral on behalf of someone, please ensure you have their permission before doing so. 

If you are referring someone to our hearing loss service the person MUST have been issued with and wear a working hearing aid.
